Before I crank down on the axle, I put a rag between the chain and sprocket and rotate the wheel. This brings the wheel tight against the adjusters and prevents it from lossening later. Once the axle has been tightened, rotate the wheel back and remove the rag. It takes a while to get used to this because it makes your chain looser than you thought. But once you adjusted it correctly it should stay that way. And once you tighten the axle, snug up the adjusters again. I've had them come loose and fall out on the road before.
